I hereby apply for membership of FORTH Club. If accepted, I agree to be bound by the below rules. For a more comprehensive list of our bylaws, click here.

1. All sections of the application are complete.
2. All information shall be treated in confidence. 
3. Membership commitment is annual via monthly payments. A Member's annual commitment will auto-renew every 12 months, unless cancelled.
4. Both membership dues and initiation fees are subject to applicable taxes and service fees.
5. FORTH Club applicants must be 21 and over.
   1. The bar lounge & terrace is only accessible to Members and their guests 21 and over.
   2. The pool and fitness center allow for Members' guests of all ages. All Members' guests under the age of 16 must be supervised at all times.
6. FORTH Club Members are allowed three (3) guests during each visit to the bar lounge & terrace, one (1) guest at the pool, and twelve (12) guests annually to the fitness center. All Members' guests must be accompanied by the Member. FORTH Management reserves the right to hold 'Members Only' events. 
7. FORTH Club current membership dues and initiation fees for Individual & Couples Applicants are as follows:
